Online Registration Steps


1. Goto Registration Website CLICK HERE

2. Click on the “Registration” tab across the top

3. Click either the “Player Registration” or “Coach Registration” (depending on who you are registering).

4. If your child has played before use “Option 2”. If no member of your family has played before use “Option 3”.
(If “Option 2” is used, there will be a page titled “We found a match”. Please be sure to click next to a parent’s name on this page.)

5. Select the parents and players you are registering.

6. Review the information to be sure the correct player is registering in 2009-2010.

7. Select the play level. If you are playing AAA, select competitive. If you are playing AA, select District.

8. On the player information page double check all of the information, then fill out the elementary school that you live closest to, even if that is not the school that you attend.

9. Review the registration summary

10. Print the forms and sign them. The notary on the medical form is not required. Turn the forms into your team administrator with any other information they are requiring. Please print 2 copies of the registration form for each player - there are two places on the registration form that need to be signed.

NOTES:

You must have Adobe 8.0 in order to print the forms.

Anytime you want to go back into to edit or look at your records, log into the URL address and enter you username and password on the left. Do not go back into the registration tab.

Reminders:

**Each member of a family will need to register under the Club that they will be playing for. The links on this page are ONLY for HC Storm Registrations

**Be sure the Primary Contact person is a parent.

**If your child’s name comes up two times, please make sure only one of the names is checked.

**A registration form, medical release form and parent/player conduct form will automatically print at the end of the registration process. It is OK if only one parent’s information prints out. Please be sure to sign both sections of the registration form, before turning it in to your team administrator.

**If you have problems with on-line registration, please report what problem you had, and go to www.uysa.org to print out a registration form to fill out and turn in.
